UnitedHealthcare's Behavioral Health Gold Card: Who Skips Prior Auth Now
Since October 1, 2026, some behavioral health practices no longer send clinical prior authorization requests to UnitedHealthcare for intensive outpatient, partial hospitalization and psychological testing. The catch: it only applies to practices that earned a Gold Card, and only for a short list of codes.
Who qualifies
Optum Behavioral Health measures each practice by its Tax ID (TIN). To earn a Gold Card, a practice has to meet all of these:
In network for at least one UnitedHealthcare line of business: commercial, Medicare Advantage, Medicaid or Individual Exchange.
At least 10 eligible prior authorizations for Gold Card codes in each of two back-to-back 12-month periods.
An approval rate of 92% or higher, after appeals, in each of those periods.
For the waivers that started October 1, 2026, the review period ran from April 2, 2024 through March 31, 2026. The program covers behavioral health services under UnitedHealthcare commercial, Individual Exchange, Medicare Advantage and Community plans.
Which services it covers
Intensive outpatient (IOP): revenue codes 0905 and 0906, H0015, S9480
Partial hospitalization (PHP): revenue codes 0912 and 0913, H0035, S0201
Medicare and D-SNP PHP and IOP: revenue codes 0914, 0915, 0916 and 0918, billed with the primary CPT code and condition code 92 (IOP) or 41 (PHP)
Psychological testing: 96130, 96131, 96136 through 96139, 96146
S9475 (ambulatory substance use disorder treatment), H2001 (Missouri only) and H2033 (Virginia only)
What doesn't change
Gold Card practices still complete Optum's Advance Notification process for these services. It replaces the medical necessity review, not the notification.
Every other behavioral health service that needs a prior auth still needs one.
You still verify eligibility and benefits before every service.
Some states have their own prior auth rules for these services, and those still apply.
What to do now
Find out whether your TIN has a Gold Card. Optum Behavioral Health handles the reviews and the notifications. The window to dispute this year's status closed October 1.
If it does, switch those services to advance notification in your intake workflow, and keep auth tracking on everything else.
Watch your approval rate on these codes. Optum reviews every practice once a year, and new status takes effect each October 1.
Payer rules like this change all the time. Korps tracks them for practices in every specialty, so your team doesn't have to.
